Master Facility Mapping Table (MFMT)
Please use the Master Facility Mapping Table to help locate the clinic of your interest. The MFMT has data from multiple systems including but not limited to,
ORCHID
eConsult
ePrescribe
Legacy Affinity
ELM (empanelment data only)
CACTUS
Etreby
Community Partner Data
Before using the MFMT, know which data source you will be using. If you are unsure of this information, please consult with your data analyst.
The MFMT is accessed through DHS Remote Desktops using your E or C #.

3. Locate MFMT under resources (Download the excel to be able to filter)
4. Use the Legends and Macros tab to help you filter the main table (updated_XXXX2020)
Please note the following:
Cactus Location refers to ELM
Cells highlighted in blue are naming conventions that came directly from the source system
5. In the main table (updated_XXXX2020), each of the sources has their unique attributes, so please don’t use the table without first filtering by source. For example, if you are filtering by ORCHID, Attribute 1 will give you appt. type-Facility (schedule), whereas Econsult, Attribute 1 will give you provider group.
a. Select your source by going to the updated_XXXX2020 tab and follow the steps below
i. On Sources, use the drop-down arrow → Click on Filter → select only one source at a time
6. Now that you have filtered the main table by a source you can continue to narrow your search by filter the Level and Attribute columns.
a. Levels represent facilities that have been mapped; levels 0-1 include,
i. Level 0: DHS, DMH, DPH, or some non-county locations
ii. Level 1: contains specific hospitals, non-county community partners, and non-county hospitals
iii. Level 2 (locations): Individual clinic location, individual health center, individual hospital
iv. Level 3 (more granular): Specific areas within a clinic: Ex. Harbor Hospital → Subcategories of physical locations
b. To help you identify the Attribute use the Legends and Macros tab
Data request example
Data request: I am looking for a list of Rheumatology patients in 4P1 clinic and LAC+USC medical center
Filters Include
Data Source: Orchid
Level 0: DHS
Level 1: LAC + USC
Attribute 6 (Subspecialty): Rheumatology
Once you have the filters in place the Clinics will populate in the corresponding columns
