Add the DHS Shared drive access (S:drive) file path information to the form. The file path and instructions will be provided to you by the Informatics and Analytics Core team. Example: (\\10.106.193.41\Planning)(K:)(IRB NUMBER - PI NAME - TITLE)

NOTE: If you are not a DHS Investigator (i.e. Research Associate, Resident, Subinvestigator) but your IRB indicates that you are a research staff personnel that is permitted to access identifiable data, your DHS Investigator must submit a ticket to Helpdesk@dhs.lacounty.gov requesting your individual access to the DHS Shared Drive.
