Please fill out a detailed description of your IRB and research data request in this form. Submit this form in conjunction with IRB documentation and other approval documents (i.e. letter of approval with categorization score) when submitting a ticket for a DHS Data Request. Be sure to include “Research Data Request” and IRB number in your email subject to Helpdesk@dhs.lacounty.gov in order to open a ticket to the DHS Informatics and Analytics Core.
