Helpful tips for Investigators completing the network access request form:

  • Request service and management permission

    • Check “Add”

    • Add the DHS Shared drive access (S:drive) file path information to the form. The file path and instructions will be provided to you by the Informatics and Analytics Core team. Example: (\\10.106.193.41\Planning)(K:)(IRB NUMBER - PI NAME - TITLE)

    • Supervisor name and signature (can be your County supervisor or sent to ccatignas@dhs.lacounty.gov for sign-off by Informatics Core Manager Daniella Garofalo)

  • NOTE: If you are not a DHS Investigator (i.e. Research Associate, Resident, Subinvestigator) but your IRB indicates that you are a research staff personnel that is permitted to access identifiable data, your DHS Investigator must submit a ticket to Helpdesk@dhs.lacounty.gov requesting your individual access to the DHS Shared Drive. Please be sure that the IRB and protocol are attached to the ticket to confirm that you are permitted as research personnel to view identifiable data.
